Flu shots for Twin Cedars Schools will be Tuesday, October 8. We will arrive at 12:30pm.
I have attached the flu form to this email. Please send the attached consent form district wide via email/website so all parents receive it. Flu forms are due on Tuesday, September 24, 2024. The parent/guardian will be responsible for filling out the consent form and returning it to the school nurse. I will pick up completed forms/payment from the schools on Thursday, September 26.
Please let us know if you would like a paper copy.
For students who have private insurance that cover immunizations, there is a $30 charge for the flu shot. For those on Medicaid, no health insurance, underinsured, or American Indian/Alaskan Native, there is no cost because we are part of the Vaccines for Children program, and they cover the cost for those who are eligible. Insurance information is asked on the consent form.
